Investment Account Forms

OPEN A NEW FOUNDATION ACCOUNT

To open a new account at the Wisconsin United Methodist Foundation, your organization should complete and submit the following two forms to the Foundation:

Send us your completed “Information Sheet” and a signed copy of the “Trust Agreement for Common Fund” form. We will also sign the Trust Agreement and return it to your organization along with a completed “Memorandum of Understanding” which summarizes the instructions you provided on the Information Sheet about how this account should operate.

DEPOSIT FUNDS TO YOUR ACCOUNT

Mail us your check for deposit to your account at the Wisconsin United Methodist Foundation. If your organization has multiple accounts at the Foundation, you can include a completed deposit slip with your check, to insure that your deposit is directed to a specific account:

REQUEST WITHDRAWALS FROM YOUR ACCOUNT

To request funds from your Foundation account, please complete a withdrawal request form. Obtain an appropriate signature on the form, and either mail, e-mail, or FAX the form to the Wisconsin United Methodist Foundation. Withdrawals are only processed at month-end (12 times per year). We must receive your completed withdrawal request form in our office by the 25th of the month in order to process it for the upcoming month-end. RECEIVE DIRECT DEPOSITS, INSTEAD OF CHECKS

To have all future withdrawals from your Foundation account electronically deposited directly to your organization’s bank account, please complete and submit the following form to the Foundation:

VIEW YOUR ACCOUNT ON-LINE

If you are still receiving printed quarterly statements, please complete and submit the following form to the Foundation so that you can view your accounts online monthly:

CHANGE AN EXISTING ACCOUNT’S INVESTMENT ALLOCATION

To change the percentage allocation of underlying assets of an existing account (percentage invested in Equities, Fixed-Income Investments or Loan Pooled Assets) please complete and submit the following form to the Foundation:
